The pilot’s failure to maintain control of the airplane during the landing.
Failure of maintenance personnel to inspect the crankshaft after two previous propeller strikes, which resulted in failure of the crankshaft and a total loss of engine power. Contributing to the crankshaft failure was insufficient torque applied to the thrust nut.
The pilot’s improper fuel management before landing, which resulted in fuel starvation and the subsequent total loss of engine power during a subsequent go-around when the fuel became unported during the climbing turn.
